Your Graduate School Application requires the following items:
- Three (3) Letters of Recommendation
- a statement of professional intent/interest
- GRE Scores
- Official Transcipts
Detailed Steps Through Online Application Process
The following steps will guide applicants through the online application process. Applicants will be required to submit university transcripts and admissions test scores as part of the application process.
Visit theUSU online application at this link. Once there, follow these steps:
- Click “Online Application”
- Click “First time user account creation”
- Create Login ID and PIN
- For application type select “School of Graduate Studies”.
- Select the term that you are applying to begin taking classes. Provide your full name.
- Continue with online application. Fill in all required sections. Click “Finish Later” to save and return to application later. Click “Application is Complete” to submit application.
- The online application will ask for the following information:
- Demographic Information: “Legal Name”, “Permanent Address”, “Mailing Address”, “Personal Information”, “International Information”, and “Contact Information”.
- Full names and email addresses of three individuals who will write your letters of recommendations: “Planned Course of Study”.
- College name, City, State, Attendance dates, Degree received: “Previous College”.
- Admissions test name (MAT, GRE), score received, and date taken (MM/ YYYY): “Test Scores”.
- Place of legal residence and employment information for the past 5 years: “Utah Residency Questionnaire”.
- A statement of your professional intent/interest (no word limit). This will be used to assess your writing skills and reason for applying:."Additional Information".

M.S. in Psychology from School Counseling Program
Ed.S. in Psychology from the School Psychology Program
Ph.D. in Psychology from the Experimental and Applied Psychological Science (EAPS) Program
Ph.D. in Psychology from the Combined Clinical, Counseling, School Psychology Program
